Case Resource Manager
The Bureau, Marshall, Putnam County Health Departments, is hiring numerous Case Resource Manager to support Illinois Public Health's efforts in performing COVID-19 contact tracing.
Position Summary
BMPCHD is organizing teams of community contact tracing staff across our Tri-County, as a key strategy for preventing further spread of COVID-19.
The Resource Manager will work remotely to conduct a virtual needs check and connect at-risk COVID-19 positive patients and people in quarantine to community resources.
Essential Duties & Responsibilities
Identify social services and community resources that would benefit COVID-19 patients and their families, such as food banks, mental health services, visiting nurses, and other community programs helping patients in need and/or people in isolation or quarantine. Conduct virtual needs checks with patients and their contacts and connect them to community resources. Assist the Contact Tracers with phone calls to COVID-19 patients to provide support and gather information and help patients recall everyone with whom they have had close contact during the timeframe while they may have been infectious. Then, call the contacts to discuss their potential exposure. Communicate with contacts in a professional and empathetic manner. Collect and record information on symptoms into a Client Resource Management platform CRM. Maintain contact with supervisor. Conduct other related duties as assigned.
Minimum Qualifications
Bachelor's degree in social work, public health or a related field, or 4 years of related professional experience. At least 2 years of related experience. Experience in social work or public health preferred. Familiarity with and experience working in Illinois health care system. Experience working with Bureau, Marshall, and Putnam County community resources and social services that would be beneficial to COVID-19 patients and their families. Excellent interpersonal skills and ability to interact professionally with culturally diverse individuals during a time of crisis and distress. Ability to communicate verbally in a professional and empathetic manner. Excellent organizational skills. Good critical thinking skills. Ability to handle confidential information with discretion and professionalism. Proficiency with computer software and ability to learn how to use CRM platform. Work experience and/or lived experience with communities experiencing/at risk of disproportionate COVID-19 impact. Ability to speak, read, and write in English. Familiarity with our Tri-County geography.
Desired Qualifications
Fluency in a second or in multiple languages a plus (specifically Spanish). Please indicate languages spoken in your application and/or cover letter. Preference will be given to residents of Bureau, Marshall, and Putnam County.

This is a full-time (40 hours per week) position. This is a remote position based in Bureau, Marshall, and Putnam County.
