Public Health Nurse, Registered Nurse or Social Worker for Home and Community Based Services.

Full Time Public Health Nurse, Registered Nurse or Social Worker Associate’s or Bachelor’s Degree in Nursing with current MN RN License and/or PHN Certificate desired or Bachelor’s in Social Work or closely related field. 2 years experience preferred in Home and Community Based Services required. Job duties include health and wellness assessments, long-term care consultation, case management/care coordination, and computer documentation. Starting pay range $26.92 to $28.27 per hour depending on qualification. Prior public health experience and bilingual abilities desirable but not required.
